IMPORTANT: Before selecting the PDF of the form you need, please read the following ways you can use PDFs of forms on this site:

Option 1: Using the form directly from the web site.
|
When you left mouse click on a form it will open. You will then be allowed to complete the form and print it only. You will not be able to save the form from here. When you exit from the web site the completed form is not saved or retained anywhere.

NOTE: You must have purchased and installed the complete Adobe Acrobat software before continuing with Option 2.

Option 2: Export the blank form from the site to your computer.
|
- Right click and hold on the link to the form you wish to select until a menu appears.
- Choose Save Target As from the menu.
- Select the drive/folder you wish to save to.
- Keep the existing filename and file format (extension) as PDF.

Using the exported form saved to your computer.
|
- Open the form from where you saved it on your computer.
- Complete the form DO NOT CLICK ON SAVE.
- Click on File > Export > Form Data
- Select drive/folder you wish to save to, you can keep the existing filename or type in a different one. Keep the default extension FDF.
- Click on Save. (The form is now saved with the data and can be opened and edited).

The original PDF file/form with the information will still be open. When you close this file you will get the message Do you want to save changes click NO. This form will be saved without any information and will be ready to use again.

- Select Download Link to Disk.
- Select the drive/folder you wish to save to.
- Keep the existing filename and file format (extension) as PDF.
- Repeat until you export all the CCF forms you desire and then close the web site when finished

- Open the form from where you saved it.
- Complete the form DO NOT CLICK ON SAVE.
- Click on File > Export > Form Data
- Select drive/folder you wish to save to, you can keep the existing filename or type in a different one. Keep the default extension FDF.
