Using Online CCSD Forms

Most forms are in PDF format. To open, complete and print a CCSD form, you must have Acrobat Reader installed on your computer. However, to save your data within the form, you must have Adobe Acrobat 5.0 or highter installed on your computer. Reader will only allow you to view, fill out and print the form.

Software Plus has been named our Microsoft and Adobe Software Reseller. Beginning August 18, 2003 most orders for Microsoft and Adobe software may be purchased using the STOR (Quickship) system. A catalog with the STOR (Quickship) software titles is available from the Purchasing Department. You may view the catalog on the Purchasing Web Site, or request an electronic copy via Interact. Windows Instructions
IMPORTANT: Before selecting the PDF of the form you need, please read the following ways you can use PDFs of forms on this site:

Option 1: Using the form directly from the web site.
When you left mouse click on a form it will open. You will then be allowed to complete the form and print it only. You will not be able to save the form from here. When you exit from the web site the completed form is not saved or retained anywhere.
NOTE: You must have purchased and installed the complete Adobe Acrobat software before continuing with Option 2.

Option 2: Export the blank form from the site to your computer.
  1. Right click and hold on the link to the form you wish to select until a menu appears.
  2. Choose Save Target As from the menu.
  3. Select the drive/folder you wish to save to.
  4. Keep the existing filename and file format (extension) as PDF.

Using the exported form saved to your computer.
  1. Open the form from where you saved it on your computer.
  2. Complete the form – DO NOT CLICK ON SAVE.
  3. Click on File > Export > Form Data
  4. Select drive/folder you wish to save to, you can keep the existing filename or type in a different one. Keep the default extension FDF.
  5. Click on Save. (The form is now saved with the data and can be opened and edited).
The original PDF file/form with the information will still be open. When you close this file you will get the message “Do you want to save changes” click NO. This form will be saved without any information and will be ready to use again.


Macintosh Instructions

  1. Click and hold on the link to the form you wish to select until a menu appears.
  2. Select Download Link to Disk.
  3. Select the drive/folder you wish to save to.
  4. Keep the existing filename and file format (extension) as PDF.
  5. Repeat until you export all the CCF forms you desire and then close the web site when finished

Using the exported form saved to your computer.
  1. Open the form from where you saved it.
  2. Complete the form – DO NOT CLICK ON SAVE.
  3. Click on File > Export > Form Data
